Can a simple tightening of the mask strap cause mask squeeze? My girlfriend did fine first dive, then tightened her mask before second and got hit big time....nothing else different!

Yes. That could easily cause the squeeze by not allowing for easy equalizing of the airspace. Remember, the mask strap only needs to be tight enough to keep it on her face.
