How do I go about getting a replacement card?
I lost it and need to get a another.

Either goto the store you got your c-card at and have your instructor fill out a replacement form, or Call PADI at 1-800-729-7234, or you can goto http://www.padi.com and download a replacement form and mail it to PADI. The cost is $25.00
