Department of Transportation's Research and Special Programs - Tank Safety Advisory The U.S. Department of Transportation's Research and Special Programs Administration (RSPA) announced Sept. 6 potential dangers with certain improperly tested self-contained underwater breathing apparatus (SCUBA) tanks, self-contained breathing apparatus (SCBA) tanks, and other compressed gas cylinders. The RSPA determined that Underwater Adventures, located at 400 W. Magnolia, Leesburg, Florida, marked and certified an undetermined number of cylinders as being properly tested in accordance with Federal Hazardous Materials Regulations (HMR), when the cylinders were improperly tested. In addition, Underwater Adventures marked an undetermined number of cylinders with a Retester Identification Number (RIN) that belongs to another company. A hydrostatic retest and visual inspection of a cylinder are used to verify the cylinder's structural integrity. If the hydrostatic retest and visual inspection are not performed in accordance with the HMR, a cylinder with compromised structural integrity may be returned to service when it should be condemned. Serious personal injury, death or property damage could result from rupture of a cylinder. Cylinders that have not been retested in accordance with the HMR may not be charged or filled with compressed gas or other hazardous material. Underwater Adventures failed to keep records of its retest and visual inspections of cylinders, making it impossible to determine the number of cylinders that the company improperly tested. These cylinders may pose a safety risk to the public. The cylinders in question are stamped with one of the following two RINs: B095 or B059. The markings appear in the following pattern: B 0 Anyone who has a cylinder that has been serviced by Underwater Adventures and that is marked with RIN numbers B095 or B059 and stamped with a retest date after 1986 should consider the cylinder unsafe and not fill it with a hazardous material unless the cylinder is first properly retested by a DOT-authorized retest facility. Filled cylinders (if filled with an atmospheric gas such as oxygen) should be vented or otherwise safely discharged, and then taken to a DOT-authorized cylinder retest facility for proper retest to determine compliance with the HMR and their suitability for continuing service. Under no circumstance should a cylinder described in this safety advisory be filled, refilled or used for any purpose other than scrap, until it is re inspected and retested by a DOT-authorized restest facility. The matter is being investigated by the RSPA's Office of Hazardous Materials Enforcement. Aeris has recalled

Evaluation has shown that a specific batch of HP (High Pressure) Seats contained material that can deteriorate and fail to properly perform its mechanical sealing (shut off) function, causing an uncontrollable free flow which could result in serious injury to or death of the user. In cooperation with the U. S. CPSC (Consumer Product Safety Commission), ROMI is initiating a voluntary recall of all affected AT400 First Stages. The affected First Stages include those that were shipped to AERIS Retailers from the factory between May 1, 2006 and November 15, 2006. Other Diaphragm type First Stages that received annual service between May 1, 2006, and October 22, 2007, may also be affected. NOTE: Due to the random nature utilized to assign SNs (serial numbers) to First Stages, there is only a small quantity of affected units within the large range of SNs. Since the extent of listing the actual SNs of all affected units would be impractical and not user friendly, AERIS has created a data base on the diveaeris.com web site that product owners and retailers can easily access to determine whether a specific First Stage is affected and if it has received the retrofit. If you have an affected unit, you must discontinue further use of it and take it to the AERIS Dealer where you purchased it, or a local Authorized AERIS Service Center, for a component retrofit at no charge. If you no longer own the Diaphragm type Regulator First Stage, please forward this letter to the new owner or have them contact us directly. WARNING: DO NOT CONTINUE USE of the Regulator First Stage until it receives a Retrofit. Only Authorized AERIS Dealers and their trained service technicians must perform this Retrofit. Non authorized persons who attempt the Retrofit procedure risk voiding the product’s warranty and rendering the product dangerously unsafe for SCUBA use. It has come to our attention that a number of the new ATX regulators produced in September/ October 2001, have a potentially serious defect. The problem is related to the 2nd stage front cover of the ATX100, ATX200 and all ATX fluorescent yellow octopus regulators, this includes the Stage Three sets. Your local Apeks dealer / distributor has been informed of the situation and you should take your regulator to them immediately for inspection. May 22, 2007 There maybe a potential problem with the yoke clamp screw on any yoke clamp type regulator with the serial number starting from 7010001 – 7053528. Please return your yoke clamp screw to your nearest Apeks dealer for inspection, do not dive with the regulator until it has been inspected. If it is not possible for you to return the yoke clamp screw for inspection, then please contact AquaLung/Apeks’ Customer Service Department for assistance. Aqua Lung This is limited to those 2nd stages/octopus that have not undergone an authorized annual service. It has come to our attention that an incorrectly assembled 2nd stage has been purchased. If you have an Apeks regulator or octopus that has never had an authorized annual service, regardless of how recently purchased, please take the regulator/octopus to your Apeks dealer to be visually inspected. All Apeks dealers and distributors have been made aware of this issue. Do not dive with the regulator until it has been visually inspected. Apeks apologizes for any inconvenience this may cause. Atomic Aquatics Inc., of Huntington Beach, California, has announced a recall of all T1, B1 and Z1 models due to the possibility of a failing first stage spring. It is a voluntary recall announced by Atomic President Dean Garraffa, who added that there have been no reported incidents of in water failure or diver injury. The fault was detected in three regulators undergoing regular servicing when cracked springs were noticed. The recall covers about 3,500 T1, B1 and Z1 regulators manufactured between May 11, 1999, and October 27, 1999, with serial numbers # A07751 - A11276. The serial number can be found under the mouthpiece of the second stage. Atomic - Cobalt Computer HP Sensor Leak Atomic Aquatics has decided to initiate a recall on the Cobalt Dive Computer after discovering a possible assembly problem that could cause the high pressure sensor to leak. Although we hope the percentage of units with this problem is small, the defect is serious enough that they feel it is necessary to recall and inspect as many units as possible to ensure customer safety and satisfaction. Fortunately the defect is easy to identify and we hope that you will cooperate with Atomic to minimize the inconvenience to the you, the customer. The computer may be returned to Atomic for inspection, but doing it at the retail store is simple and will expedite the process for your customer. Having a Cobalt in the affected serial number range does not mean it is defective, but that it should be inspected for the defect. Any computers found with this defect must not be used and returned to the factory for repair or replacement. Affected units: Affected units that require inspection can be identified by the first four digits in the serial number. To view the serial number, turn the unit on and go to “Main Menu/ Settings/System Information." For more details, contact Atomic Aquatics. There is problem in the Cobalt date checking routine that prevented firmware versions 1.10 and later from automatically updating the year to 2012. Please check your Cobalt's date. If it is incorrect, change the year in the "Set Time and Date" screen to 2012, so your dive log will record new dives with the actual date. If left in year 2011 and there is an existing dive in the log just previous to the new dive, you might experience shorter than normal no-deco times as the Cobalt will treat it as a repetitive dive. Do not attempt to change the date if you are between dives. Always wait at least 24 hours after your last dive before changing the date or time or you will lose the repetitive dive calculation. This will be corrected in the next firmware release. In the event you are unable to complete the date change to 2012 please contact Atomic at (714) 375-1433 or take it to your local Authorized Atomic Retailer. Aqualung's ABS Octopus Exhaust Grid Replacement Aqua Lung has determined that it is possible, under certain circumstances, for
one or both of the exhaust valves on the ABS Octopus to become inverted. Inverted
means that the edge of the exhaust valve can get lodged between two of the
spokes of the seating surface. This could happen during the after-dive rinsing
process if the valves are hit with a stream of high-pressure water from a
garden hose or if the valves hit the water just right during an entry. The
result of having one or both valves inverted is the inability to completely
seal out water. This could lead to an aspiration of water, which in turn could
lead to panic, injury or death. The valves will not invert during the course
of a dive. To protect the valve from inverting, Aqua Lung has redesigned the
exhaust grid that covers the exhaust valves. Aqualung's Powerline Airway Ribbed hose Replacement Aqua Lung has received reports that some of the ribbed hoses (Fig. 1) on the Powerline Airway have been tearing at the top near the dual exhaust valve. The problem has been traced to an error in the molding process from our supplier that occurred over a three-month period between 2001 and 2002. The best way to confirm whether or not you have one of the questionable hoses is to look at the serial number on the lower unit of the Powerline (fig. 2). If the serial number falls within the range of M48H to M52H (M48H, M49H...M52H), or M01I to M08I (M01I, M02I...M08I) and is the original hose, then it should get replaced. This affects both SeaQuest and Aqua Lung BCs utilizing the Powerline inflator. If the ribbed hose were to tear near the top, the BC would not be able to hold air and a loss of buoyancy would occur. This could result in the need for the diver to ditch their weight pockets or rely on their buddy for assistance. In extreme cases it could result in result in panic, injury or death. If you have a spiral hose that falls within the serial number range, please have the hose replaced by this authorized Aqua Lung retailer Aqualung - SureLock II Weight Pocket Handles Aqua Lung is conducting a voluntary recall of SureLock II weight pocket handles. SureLock II is the mechanical weight release system found on most Aqua Lung buoyancy compensators (BCs) beginning in 2009. Aqua Lung is concerned about the rubber handle pulling off of the plastic SureLock II weight pocket assembly. If this were to occur during a dive, the weight pocket would be left engaged inside the BC. There are no known occurrences of this happening underwater, and there have not been any reported injuries, but Aqua Lung believes that a recall is the safe and prudent thing to do. Aqua Lung has made a revision to the handle assembly that strengthens it. A revised handle is easily recognized by a thickened band of rubber at the base of the handle.
Consumers should immediately stop using their BCs with Sure Lock II weight pockets. Bring the pockets to your nearest authorized Aqua Lung Dive Retailer or Dive Center. They will be able to replace your handles for you while you wait, as changing handles is a quick process. The replacement is made under warranty and you will not incur any charges. Aqua Lung has determined that some S.O.S. units (PN 394157 and 394057) sold since Sept 2002 may have a faulty component. The component is known as an “elbow,” and is used to transfer air from the BC to the safety tube. Some elbows have exhibited signs of degradation leading to cracking and/or breakage. If this were to occur, air may not successfully transfer from the BC to the safety tube, thus preventing the tube from inflating. Furthermore, if a diver were inverted, making the attachment point the “high spot,” air could also escape from the BC. The leak may prevent the BC from “fully inflating.” Although the BC will still hold air when vertical, some air will escape at the bottom and the BC will provide less buoyancy. Aqua Lung would like all owners of S.O.S. units to carefully inspect them. Elbows that have the potential to degrade are easily identified by a molding gate mark located on the outer bend (see photo.) To date, there have not been any reported accidents or injuries. If you have a unit with such a gate mark, we ask that you stop using the product. Detach it from the BC and return it to your nearest Aqua Lung Dealer. Aqua Lung asks all the owners of TITAN DIN regulators whose serial number is lower than 6062501 or TITAN/CONSHELF DIN adaptors that are marked 300 BAR MAX to return their regulator to their Aqua Lung authorized retailer for an upgrade. The corrective action is replacement of the brass DIN HANDWHEEL RETAINER by another stainless steel DIN HANDWHEEL RETAINER marked 300 BAR MAXI. This replacement is made under warranty. All the TITAN DIN regulators whose serial number are above 6062501 and\or whose DIN handwheel retainer is marked 300 BAR MAXI are assembled with a stainless steel DIN handwheel retainer and are not affected by this recall. BARE was recently contacted by one of their customers who identified an issue with the integrity of the taped connection between the neoprene boot and the fabric. Under extreme circumstances, when the diver pushes his/her feet through the leg of the suit and into the neo boot/sock it is possible to come completely free of the leg. You must discontinue all diving activities with your BARE drysuits until getting it retrofitted by sending your drysuits into BARE. Bare - SI TECH Drysuit Inflation Hoses Bare is recalling all SI TECH Drysuit inflation hoses that were sold between: June 1, 2006 - February 29, 2009. Bare has been informed by it's supplier that they have received isolated reports that the flow restricting insert may malfunction by dislodgment from the connection valve, potentially causing an obstruction of air flow. Bare - Voodoo, Blackjack, Blackwing or Curve Buoyancy Compensators July 14, 2010 BARE is voluntarily recalling all buoyancy compensators with that fall between the serial number range XX0908XXX and XX1208XXX (first two digits of the serial number represent month, second two digits represent year) were manufactured between September 2008 and December 2008. BARE was recently contacted by one of our customers who identified an issue with the threaded seal rings that connect the left shoulder rapid exhaust valve and dump valves to the BCD bladder. Under extreme circumstances, when the diver pulls on the inflator hose or release cord to release air from the bladder, the threaded seal ring may crack or break. BARE has issued a voluntary recall of all BCDs to correct the potential problem with the threaded seal ring. You must discontinue all diving activities with your BARE BCD until getting retrofitted by sending your BCD into BARE. Locating your Serial number:

The illustration below identifies the serial number on this product as XX1108XXX (manufactured in November, 2008). In this case, the buoyancy compensator falls within the recall range and must be returned to BARE for an upgrade to the threaded flange.
Options for sending in your BARE VOODOO, BLACKJACK, BACKWING or CURVE buoyancy compensator for servicing: Brownie's Third Lung - Hookah Air Compressors While few real divers use a hookah -- air supplied through hoses connected to a compressor either back on the boat or floating in a buoy -- their kids might. So be aware that Brownie's Third Lung has problems with components in some models of its compressors that can cause premature wear and tear, and also stop air flow. Brownie's will inspect, repair and upgrate affected compressors at no change and renew the warranty for another year. The recall is for compressors manufactured since July 2007 with serial numbers 14421 to 15715 (that's for models F280X, F390X, C270, C390, CTD390) and serial numbers 14344 to 15762 (models E250 and EC2). The serial numbers are printed on a metal tag on the rim of the yellow storage case or, for commercial models, on the deck. Cressi-Sub - Buoyancy Compensators The U.S. Consumer Product Safety Commission announces the following recall in voluntary cooperation with Cressi-Sub. Consumers should stop using recalled products immediately unless otherwise instructed. A slow leak from the shoulder exhaust caused by expansion of an internal cable housing could result in slow deflation. This could impact the diver’s ability to control buoyancy. Buoyancy Compensator devices provide buoyancy control for scuba divers by allowing them to inflate or deflate the devices. The recalled buoyancy compensators have model numbers J107, J113, J115, and J119. The numbers appear on the top right-hand corner of the unit (on the shoulder). "CRESSI" is written on the buoyancy control devices. Customers can return the product to a Cressi-sub-authorized dive shop to have the product retro-fitted with a free replacement internal cable housing. Buoyancy Compensator devices with a black retaining ring around the yellow inflate button on the inflation mechanism, and an adjoining gray cover assembly (bypass case), which screws onto the inflator, are subject to this recall and must be retro-fitted. Dacor: Dacor - Darwin Air and Darwin Consoles URGENT SAFETY NOTICE DACOR DARWIN AIR AND DARWIN CONSOLE DIVE COMPUTERS If you have recently purchased a Dacor Darwin Air or Darwin Console Dive Computer, please read the following safety notice regarding the recall of certain computers. HTM Sport S.p.A., Manufacturer of Dacor Dive Computers, has discovered that during production, a small number of Dacor Darwin Dive Computers were improperly calibrated, resulting in erroneous calculations displayed regarding "No Deco Time", "Deco Time", "Air Time", "Desat Time" and "No Fly" times. Products affected have the following serial numbers:
Darwin Console Computers: Serial Numbers between 8503 and 8592 Note: This will not affect computers shipped by Scuba.com after February 28, 2005. If you own one of the Darwin Computers with a serial number listed above, DO NOT USE IT! For more details, contact Dacor: If you purchased a Dacor Supreme SPG or SPG console after February 1, 2001, a defect may exist with the high-pressure hose for the Supreme SPG involving the pressure-fitted metal fittings at either end of the high-pressure rubber hose. These fittings may leak or become separated from the hose after you turn on your air supply. If the hose for the Supreme SPG were to fail during a dive, and proper dive planning and dive termination procedures were not implemented, serious injury or death from drowning or air embolism could occur. Dacor has initiated a voluntary recall of Supreme SPG and SPG consoles in order to replace these potentially defective hoses. Delta P Technology - VR3 Computers Delta P Technology, manufacturer of the VR3 Dive Computer, has announced that there is a small problem with computers manufactured between December 1, 2004 and February 9, 2005 (sold by Scuba.com between December 1, 2004 and April 7, 2005) the Dive Planning software when used in Imperial measurements. This does not affect the actual dive functions of the VR3. There is an easy way to check which computers have this software in them, if you turn the VR3 on and go into the OPTIONS menu, go into SETUP and in the bottom right hand corner there are four digits. If these digits read either 4714 or 4B1A in a classic screen VR3 or CF4D or CE67 in the color screen VR3. If your VR3 Computer has these numbers, do not dive with this computer and contact your nearest Delta P dealer immediately to receive your software upgrade at no charge. Dive Rite has issued a recall because they have received isolated reports that the springs in the over pressurization valve may rust and fail allowing the buoyancy compensator to lose air/gas and pose a drowning hazard. You must discontinue all diving activities with your Dive Rite Buoyancy Compensator until the free safety upgrade of the Over Pressurization Valve has been accomplished by Dive Rite or your local Authorized Dive Rite Retailer. 1. Stop using your Dive Rite BC If you no longer own this Dive Rite Buoyancy Compensator, please forward this letter to the new owner or have them contact Dive Rite at 1-800-495-1046. The Instructional Team at Scuba.com WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Diving Unlimited International (DUI) Inc., of San Diego, Calif., is recalling about 3,500 Overpressure Valves (OPV) used with Buoyancy Control Systems. The overpressure valve can stick in the open position, posing a drowning hazard to divers. DUI has received two reports of the valves sticking open. No injuries have been reported. These Buoyancy Control Systems come in two versions, a jacket style and a wings style. The overpressure valve is installed on these units. It is very difficult to distinguish the defective overpressure valves from those not affected. Therefore, all overpressure valves with red pulls are included in this recall. The jacket model has the DUI logo printed on the jacket pocket, epaulette, and weight pockets. The wings model has the DUI logo printed on the epaulette and weight pockets. The letters "DUI" are printed on the permanent hang tags found on the Buoyancy Control System. Authorized DUI dealers nationwide sold these Buoyancy Control Systems from November 1997 through November 2001 for about $590. Consumers should stop using the overpressure valves immediately and contact the DUI dealer where purchased for a free replacement. Durapro Systems, of Homosassa, Fla., is recalling about 1,955 scuba buoyancy compensator devices (BCDs). Under normal scuba diving conditions, the hoses on these BCDs can separate from the jacket, and possibly cause drowning. Durapro systems are aware of three reports of these low-pressure inflator hoses separating from these BCDs. These recalled Durapro Systems BCDs are the Cirrus, Stratus, Aruba, Slimline and Resort. The low-pressure hoses on the BCDs, part number 302, connects the first stage regulator to the BCD power inflator valve. These hoses are identified with the following markings: "PLIOVIC MULTI PURPOSE 1/4" 1/D (6.4 mm) 250 PSI WP (1.7 MPA)...MADE IN USA...GOODYEAR." Retail dive stores and a mail-order catalogue sold these BCDs from January 1996 through May 1998 for about $179 to $359. Consumers who purchased one of these Durapro Systems BCDs should stop using it immediately. The hose from the BCD should be returned to the dive store where purchased for a free replacement hose. These handwheels are unlike the units used in previous or later sales of Halcyon manifolds. In use, the defective handwheels can separate from their stainless steel inserts, making it impossible to turn the valves on or off. Use of these handwheels should be discontinued immediately and returned to Halcyon for replacement. The defective handwheels can be difficult to distinguish from good ones and should be returned if in question. We are providing details regarding the voluntary recall of select Overpressure Relief Valve (OPV) Springs. Halcyon Manufacturing Inc. in cooperation with the US Consumer Product Safety Commission has initiated a voluntary recall of select Eclipse, Evolve, Explorer, Pioneer and CCR35 buoyancy compensators (BCDs) manufactured between January, 2006 and September, 2008. In addition, select Halcyon inflatable devices (Lift Bags, SMB’s, DAM’s, Surf Shuttles and Diver Lift rafts) may also be affected. These BCDs or Inflatable Devices could develop excessive corrosion or rusting problems with the stainless steel Over Pressure Valve (OPV) spring, causing the spring to fail and the BCD or Inflatable Device to leak through the OPV. It is unlikely that most of these units will experience any problems since the potential defect is limited to a very small portion of the total number of OPV springs used during this time period. However, given the potentially serious implications all OPV springs should be checked to minimize risk or inconvenience to Halcyon customers. The recalled overpressure valve is installed on buoyancy control systems with the following brand names: Diving Unlimited International (DUI), International Divers, Ocean Management Systems, Rip Tide and Steam Machines. Brand names and logos are printed on the epaulettes and pockets. These buoyancy control systems come in two versions, a jacket style and a wings style. The date code, printed on an instructional label sewn to the buoyancy control system, is the first four numbers after the brand name abbreviation. The first two numbers indicate the month and the second two numbers indicate the year. The recall includes date codes from October 2000 through June 2001. Valves with an asterisk below the serial number are not involved in this recall. Authorized CBI distributors sold the buoyancy control systems with the overpressure valves nationwide from October 2000 through November 2001 for about $590. Mares - Guardian Dive Computers Mares America Corp. of Norwalk, Conn., is recalling about 1,200 dive computers used to determine dive depth, dive time and other automatic measurements during scuba diving. Guardian dive computers can fail to function when water leaks into the battery compartment during a dive. Also, the computer can malfunction by not automatically switching on during a dive or the computer control buttons on the computer can fail to operate properly. These failures could result in divers suffering decompression sickness, which can lead to significant injury. The Guardian dive computer comes in two models, a wrist mount and a hose mount. The computer is about 3 inches wide and 3 inches long. It is black and gray with yellow buttons. "Mares" and "Guardian" are written on the frame of the computer around the screen. Selected Mares dive shops sold the Guardian computers nationwide from January through August 1997 for about $400. Nikon - Nikonos V Discontinued From Nikon: Nikon Corporation will stop making the NIKONOS-V camera body in the near future. Regional Nikon distributors and importers can be contacted for details as to when the product will be discontinued. A customer base of divers and underwater photographers have favored this camera since it was introduced in 1984. In light of relatively low sales volumes for this niche product in an age of growing operating cost-consciousness, however, Nikon determined that it could not continue to justify the cost and effort of specialized production required to manufacture it in small quantities. We are sorry for any problems that this may cause, and ask you to understand our need to make this decision. Nonetheless, Nikon remains proud and appreciative that the NIKONOS-V has enjoyed a long, distinguished product life. Although the company has no current plans to introduce new underwater photography products, we will continue to stay abreast of the market for next-generation underwater cameras. About NIKONOS-V interchangeable lenses and accessories Although Nikon will discontinue manufacture of the NIKONOS-V camera body, it will continue to supply accessories for the NIKONOS-V, including interchangeable lenses, speedlight, close-up outfit unit, etc., at least until the end of 2002. Oceanic is recalling about 24,700 pieces of scuba diving equipment that could leak air, possibly causing divers to drown. Oceanic USA, of San Leandro, has received six reports of air leaks, along with either noise or vibration, from their Oceanic CDX first stage regulators, according to the Consumer Product Safety Commission. No injuries have been reported. CDX regulators included in the recall have serial numbers 9200001 through 9205622, 9800013 through 9801711, 0200001 through 0213294, 0D0001 through 0D3046, and 9D0001 through 9D3273. They were sold with second stage regulators: Alpha 7, Delta 3, Gamma 2 and Zeta. Oceanic - BCD Power Inflator Recall Oceanic is recalling all Buoyancy Compensators fitted with the Reliant Style Power Inflator shipped between February 1, 2004, and June 24, 2004. The BCs with the

Oceanic - Versa Pro revision 2A Voluntary Recall Oceanic has received a report of two Versa Pro revision 2A units that displayed excessive Elapsed Dive Time while being used in User Selected Digital Gauge Mode. No injuries have been reported. The Elapsed Dive Time that the units displayed when they were operating in Normal Dive Computer Mode was accurate. In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Oceanic has initiated a voluntary recall of all Versa Pro revision 2A Dive Computer modules. Other revisions of the Versa Pro and other model Oceanic Dive Computers do not display the excessive Gauge Mode Elapsed Dive Time and are not included in the recall. Oceanic Versa Pro 2A Dive Computers subject to the recall are only the revision 2A units which have serial numbers 12000 through 18176 which is located on the side of the module and can be viewed if after the front button is depressed to activate the unit, it is held depressed until the Diagnostic Mode screen completes its countdown and the Serial Number screen appears displaying the module’s SN and software firmware revision (r2A). The unit will shut Off when released. You must discontinue further Digital Gauge Mode use of your Versa Pro 2A and take it to any Authorized Oceanic Dealer, or return it directly to the factory for an Upgrade to Versa Pro revision 2B. Oceanic - FDX-10 and CDX-5 First Stage Regulators October 23, 2007– San Leandro, CA – Oceanic has been notified by its supplier (ROMI, Ent.) that they have received isolated reports of DX type Regulator First Stages exhibiting uncontrolled flow of air to the Second Stage. Evaluation has shown that a specific batch of HP (High Pressure) Seats contained material that can deteriorate and fail to properly perform its mechanical sealing (shut off) function, causing an uncontrollable free flow which could result in serious injury to or death of the user. In cooperation with the U. S. CPSC (Consumer Product Safety Commission), ROMI is initiating a voluntary recall of all affected CDX5 and FDX10 (diaphragm) type First Stages. The affected First Stages include those that were shipped to Oceanic Retailers from the factory between May 1, 2006 and November 15, 2006. Other model DX type First Stages, such as the CDX, DXi, DX3, DX4, and TDX5, that received annual service between May 1, 2006, and October 22, 2007, may also be affected. NOTE: Due to the random nature utilized to assign SNs (serial numbers) to First Stages, there is only a small quantity of affected units within the large range of SNs. Temperature displayed as expected during other conditions. *** Inserted the far left digit of the Watch Countdown Timer which remained blank with Hour Format set for 24 Hour. *** Modified the Set Watch Daily Alarm and Countdown Timer, and Free Mode Countdown Timer, functions to make them to improve ease of access and saving of the selection. ATTENTION: Poseidon Dealers and Divers A potentially serious defect has been detected in a certain production run of the Poseidon W50 Bladder used in conjunction with both the platform and BeSea model BCDs. This flaw deals with a weld in the internal air cell and it is possible that this weld could fail during a dive. Should this occur, the BCD will loose its ability to hold air and provide flotation and buoyancy control during a dive without warning, putting the diver at significant risk of serious injury or death. Both Poseidon Diving Systems and their US distributor, Waterproof Gear, Inc., are taking rapid action to correct this problem without any cost to the consumer. This defect only involves bladders identified by the lot #5445. The lot number is located on a tag permanently attached to the internal air cell beneath the zipper closure of the wing. The lot number is a 4 digit number located at the bottom of the tag to the left. Please see the graphic, and note the lot number underlined in red. These bladders did not enter the market until September 17, 2007, and any bladders sold prior to that date are not impacted and do not need to be checked. All bladders that meet the above description should be considered a serious safety risk to the diver, should not be used under any circumstances, and should be immediately returned for replacement. Although not every bladder in this lot is adversely effected by this defect, there is no way to tell which bladders are impacted. Therefore, every bladder identified from lot #5445 should be returned to an authorized dealer for replacement. Consumers who may have this product are directed to return the product to their local authorized Poseidon Dealer immediately. Do not dive with this product and do not attempt to analyze or repair any potential defect in the product yourself. The VIVA control knobs on these regulators can detach during a dive allowing water to rush into the regulator, which could result in a diver drowning. Scubapro has received six reports of the VIVA control knob detaching from the regulator, two of which occurred during a dive. The G500 second stage regulators involved in this recall have serial numbers less than or equal to 13497450. The serial number is located on the bottom face of the exhaust manifold. These products were sold in diving specialty stores nationwide from March 1997 through June 1997 for around $300. Consumers should immediately stop using G500 second stage regulators involved in this recall and contact their local, authorized Scubapro dealer to arrange for a free replacement of the VIVA control knob. Inc. Announce Recall of SCUBAPRO/UWATEC Dive Computer Consoles
CPSC and Johnson Outdoors Inc. have received four reports of the pressure gauge malfunctioning. No injuries have been reported. Only U-Line submersible pressure gauges contained in UWATEC brand Smart PRO, Pro ULTRA, and Sport PLUS dive computer consoles are included in the recall. The consoles are mostly gray in color and the brand name "UWATEC" is written on the front of each console. The pressure gauges are made in the U.S. and the computers are made in Switzerland. All of the recalled consoles have a test code that appears on the back of the gauge as follows: Test Codes: SeaCure - Sport Custom Mouthpiece
Date: March 8, 2010 SeaCure is recalling all Sport I, Sport II, and Sport III Mouthpieces that are being used on Atomic regulators (2005- present), Zeagle regulators (all models/years) or Cressi-Sub Ellipse regulators (all years). The SeaCure Sport Custom Mouthpieces with yellow inserts may be clear, blue or gray in color. The SeaCure Sport Custom Mouthpieces with yellow inserts were discontinued in 2006 and replaced by the SeaCure Hi-Flow Custom Mouthpieces. At the time, it was SeaCure’s information that very few of the Sport units were left in retailer’s inventory. However, SeaCure recently learned of three incidents involving two Sport models that were affixed to an Atomic regulator that was manufactured in 2005 or thereafter and to a Zeagle regulator. Investigation of these incidents revealed a sizing problem with certain Sport custom mouthpieces and the Atomic, Zeagle and Cressi-sub Ellipse regulators. In order to place the Sport mouthpieces with yellow inserts on these regulators, they have to be stretched to the point that the yellow insert could detach from the wall of the mouthpiece and move. This has the potential to cause a problem for the diver because the movement of the insert into the second stage of the regulator could make breathing difficult. If the mouthpiece does not contain a yellow insert, this is not a potential problem because you have the updated SeaCure Hi-Flow mouthpieces. Numbers: 000000001 ~ 030204016 Sea & Sea Underwater Photography (U.S.A.) Inc., of Carlsbad, Calif., is recalling about 7,000 underwater camera strobe lights, used as a light source for photography to replace the battery cap. If the strobe becomes flooded, gas build-up can cause the battery cap to forcefully eject off of the strobe or cause the front section of the strobe to violently separate from the back section. Pieces of the strobe can strike the user or others in the vicinity. The recall includes all model number YS-50 strobes. It also includes all model number YS-60 and YS-120 strobes with serial numbers beginning with 96 or 94. The serial numbers are written inside the plastic housing. They were sold in yellow, black and orange. Writing on the strobes includes, "Sea & Sea" and "Japan." Scuba dive centers and photography stores nationwide sold these strobes from February 1988 through March 1999 for between $500 and $800. Sea & Sea have received two reports of injury resulting from the strobes. The first incident, in the Caribbean, occurred when a repair technician had a tooth chipped as he was removing the battery cap. The second incident, in Japan, caused facial lacerations to a consumer when the strobe unit violently broke apart. Consumers should immediately stop using these strobes. Any strobe suspected of being flooded, which is indicated when the unit either stops working or is corroded, should be returned to an authorized repair center for repair. Sea & Sea is offering a free replacement battery cap with a vent that prevents the gas build up within the strobe. Sea & Sea - DX-860G Camera Date: 1/5/2007 Sherwood/Genesis - BCD Power Inflator Recall Sherwood Scuba and Genesis Scuba have announced they are recalling several BCDs in cooperation with the U.S. Consumer Product Safety Commission. Sherwood made the following statement about the recall: “A condition has recently been identified that may exist in recently manufactured Sherwood Scuba and Genesis Scuba Buoyancy Compensators (BCD) that requires immediate attention. The Power Inflator an certain model BCDs could, in rare instances, provide continuous inflation and expose the diver to potential injury if appropriate measures are not taken to operate the exhaust valve to control ascent. The condition is caused by a molding variation in a seal within inflator valve, and although this variation is not present in every Power Inflator, we are requiring that all BCDs manufactured within a limited time period be updated with new Power Inflator Valve assemblies. Suunto has found a software error in the first series of Suunto D9 dive computers.
This affects all D9 dive computers with serial numbers 4xxxxxxxx – 450000699,
software version 1.x.x – 1.2.4 including all units that have been sold
prior to December 15, 2004. Although highly unlikely, the error may under
certain circumstances cause the depth and time display to update slower than
normal causing incorrect depth and time displays. Any potential problems are
avoided by upgrading the D9 software to version 1.2.8. The software version
can be verified using the Suunto Dive Manager. Please note that the transmitter
unit has no problem and is not affected. Despite an extensive pre testing program by Suunto and by independent divers all over the world, Suunto test program has found a software bug in the first series of Suunto Vytecs (products with serial numbers 205000-223700). It has been discovered that using Vytec´s SIMPLAN function may cause problems. Although diving without using the SIMPLAN function does not cause problems
and all features of the instrument work then normally, Suunto wishes that
all users contact their local dealer, which contact information you can find

replaced with a new unit. Please note that the transmitter unit has no problems

Zeagle - First Stage Regulators Zeagle systems, Inc. is recalling 931 first stage Scuba regulators shipped between January 20, 2003 and May 19, 2003. The first stage regulators affected are the following models: FlatHead Six s/n F001391 to F001476 Zeagle regulators that have been serviced since January 20, 2003 may also
be affected. WHY: These first stage regulators may have a defective high-pressure seat (Zeagle part # 341-0108-FA) that could cause the first stage to over pressurize causing the second stage to free-flow. If left pressurized and not purged or breathed on the first stage high-pressure diaphragm may fail making it impossible for the diver to breathe from the regulator. This type of failure is most likely to occur upon initial pressurization of the regulator and is unlikely to occur underwater. The seat in question is BLUE in color, other color seats are not affected. Zeagle will replace any unused Blue seats you may have in service kits at no charge by calling Zeagle customer service department. Any regulators that have been received with these seats and any that have been serviced since January 20, 2003 using the blue seats will require that the seat be changed. Zeagle will provide all necessary parts to change the seats. The parts kit will include the new seat with O-ring and the high-pressure diaphragm. WHAT TO DO: Option 1) The regulators affected can be sent to Zeagle for repair at no charge. Option 2) Authorized Zeagle dealers can install replacement seat at their facility.
